Imo, these decisions should be made collaboratively with your vet. The initial vet visit is important for a lot of reasons, and it should be done really soon after bringing home kittens. Bring the vaccination documents that you got from the breeder, along with any other medical history documents, get an initial checkup for them, and then get on a regular routine for vaccinations and such. Worming would be an excellent idea imo, but the vet can answer any questions about that.
For the unexpected types of medical expenses, pet insurance can be very helpful, and now is certainly the time to do it if you ever plan to (while they are young and don't have a history of any problems, which would become exclusions later on).
