I don't have personal experience of M&S but a colleague in the office has her two rottweilers insured with them, one of whom has needed major surgery to both knees and M&S were excellent at dealing with the claims.
From what everyone I've ever spoken to has said, PetPlan seems to be the best policy around, if you can afford it. Otherwise, it's a question of weighing up what you lose in cover by going with other companies and doing your own cost/benefit analysis based on what you can afford.
