This seems to be the #1 problem with kitties. There is a reason he's no longer using the litterbox. There's something about it that he no longer likes. New cat litter? Different type of box? Box is now too small for him? It was moved into a new area? He has to share it with other cats? Did he have a terrifying experience with maybe another one of the cats interrupting him while he was going in the box?
Since you have multiple cats, the rule of thumb is one box per cat plus one extra. Yes, if you have 4 cats, you need 5 boxes. Is that excessive? Probably! Also, if you have a multi-level home, you need boxes on each floor.
You've had him checked out, so there appear to be no medical issues! So, you need to find a way to lure him back to using the litter box. Make sure you completely clean the areas where he has peed. Pick up clothes and things -- keep the area clean from things he may want to urinate on. There is a type of litter called "Cat Attract."
If the medication you are giving him isn't working, I'd stop using it. You can always try Feliway which is supposed to calm cats and alleviate stress and you don't have to give it to them -- it plugs into the wall.
I'm no cat behaviorist, but it seems to me he's not liking having other cats in the household. I'm sensing they are stressing him out and unless you're around 24/7 and keeping an eye on the kitties, you're not going to know if something is (or has) going on. You may want to contact a bengal rescue group and see if they would take him. It appears he needs a home without other animals. It's sad, but we sometimes have to do what is best for not just us, but for the kitty too.
I hope you will try a few other things first before you give up on him, but he deserves a stress-free environment for his own happiness, too -- and so do you. Good luck.
